Adds when registration is closed

Add a new Player

• Add new player to the CUSL website using the Add a player when registration is closed selection.
  (the player will not show up on roster)
• E-mail the Club Administrator and request that your Player Card be printed.
• The Administrator will need the Team Name, Number and Player name to complete this process.
• You will be notified when the card is ready for pick up.

For added player you will need:

• signed player pass with small picture (glued) (head only)
• signed player membership form from CUSL website "Print Player registration Form"
• proof of birth date (birth cert copy or last yr's player card)

**If you have players from Indiana or Kentucky you must have the required forms their
state associations granting them permission to play in Ohio South.

• A copy of your roster
• A copy of the Roster Change Checklist
• Follow their instructions for organization of paperwork.
• Fill it out and drop it off at the club registrars house.

Add a new Coach

• Add new Coach to the CUSL website using the “Add A NEW Assistant coach to your team” selection.
  (the coach will not show up on roster)
• E-mail the Team Administrator and request that your Coach Card be printed.
• The Administrator will need the Team Name, Number and Coach name to complete this process.

For each coach you will need:

• signed coach pass with picture affixed
• signed coach membership form from  CUSL website "Print Coach registration Form"
• risk mgt form from www.osysa.com website
  • Risk Managment/Kidsafe (Employment/Volunteer) Disclosure Form
    (If you did this new last yr, you can use a copy of last yr's form)
• Coaches Code of Conduct
• A copy of your roster
• A copy of the Roster Change Checklist form
• Fill out Registration Checklist.
• Fill it out and drop it off at the Club Administrators house.
• Your Packet will be checked using this checklist to verify you are not missing anything.
